In case of an insured individuals decease, the employer/entity must notify the GPSSA through adhering to the following requirements:

 

  • Log into GPSSA’s Employer Portal
  • Select the ‘Services’ option
  • Select and initiate the ‘End of Service’ request
  • Select ‘Death’ as the reason for an end-of-service
  • Complete and submit the service form
